Adult Services Coordinator (ASC)
The Moultrie County Health Department is seeking a part-time Adult Services Coordinator (24 hours/week) to:
- Assess needs, make referrals, and ensure engagement for adult caregivers and MCHD clients.
- Comply with grant funding and program mandates.
- Expand professional partnerships and service referral systems.
- Conduct community education and outreach activities, including public presentations.
Qualifications include:

- B.A. or B.S. in community health, social work, or a related field.
- At least two years of professional experience working with adults in community health, public health, adult education, memory care, or adult social services.
- Ability to work with diverse groups of people required.
- Experience working with older adults preferred.
- Excellent communication, organizational, and decision-making skills.
